Pfizer covid-19 vaccination clinic for students 16 years old and older to be held at Hornell High School on Monday, May 10 at 5pm

Attention parents of high school students 16 years old and older; the Hornell City School District will host a vaccination clinic for students from Hornell and neighboring school districts on Monday, May 10 at 5:00 pm in their High School cafeteria.

A second vaccination of Pfizer will be scheduled for 21 days following the first vaccination. By scheduling an appointment you are agreeing to return for the 2nd vaccination.

Registrations available only to those 16 years of age or older.

A Parent/Guardian is required to physically accompany anyone 16, 17, or under 18 years of age at the date of this clinic seeking to be vaccinated.

Please bring proof of identification with you to the clinic.

After registering please complete and bring proof of completion of the NYS COVID-19 Vaccination Form. Found here: closed
